Centralizing a Nurse-sensitive Scorecard to Transform Decision-making Within a Safety Net System

Problem: Reporting nurse-sensitive indicators lacked standardization across multiple facilities in a hospital system, often grouped with performance measures not central to nursing. Comparison of progress and outcomes across common nursing unit characteristics was challenging, and reporting was confined within each hospital, limiting comprehensive review by executive system leadership.

Measurement: To balance standardizing dissemination while producing a meaningful data product, the authors selected key measures from a national nurse-sensitive quality database, aligned with prior selections for the hospitals’ Magnet Recognition designation. Sophisticated data preparation and visualization tools facilitated automation of processes for progressive reporting and agile design for future adjustments.

Analysis: The dynamic, color-coded design enabled prompt status updates, integrated with time series and rolling averages to convey long-term trends. Standardizing the presentation of common measures from multiple facilities reduced duplicate products. Streamlining underlying data preparation processes for all available indicators provides ongoing flexibility to adjust to changing data needs.

Implementation: Scorecard development enabled transformation from hospital-based silos to centralized dissemination. Executive leadership approved quarterly updates to a system-wide nursing council, individual hospital quality councils, and staff via the intranet. The traditional scorecard elements alleviated initial hesitancy to a new product, and hospital leaders quickly recognized the value of standardization.

Results/Discussion: The systematized scorecard design and data dissemination transformation have facilitated a streamlined approach to monitoring key measures routinely and over time. It offers a “one-stop shop” experience for executive leadership, transparency of successes and opportunities for improvement across the system, and shared governance in moving toward nursing excellence.
