Analysis: Analyze expected vs observed readmissions and reviewed cases to assess opportunities for process improvement.
Implementation: The nurse navigator role was implemented – power form specific for heart failure documentation of education provided. Heart failure navigator protocol was put in place. Heart Failure Booklet was created to provide every Heart failure patient on admission.
Results/Discussion: Over the course, North Kansas City Hospital recognized a decrease in Medicare FFS fiscal year heart failure readmissions to 13.9% over a two percent decrease. Continue to act as a resource and work collaboratively with the healthcare team while providing individualized heart failure education.
