COVID Modified Pediatric COLDS Scoring Tool

Problem:
Develop process for evaluation, triage, and safe reschedule of surgery for pediatric patients previously cancelled due to COVID infection.

Tool Selection:
The modified COLDS tool has been used in our institution to risk stratify patients with respiratory illness for anesthesia and surgery. This tool was modified for evaluation, triage, and decision making in COVID infected patient to assess readiness for anesthesia and elective surgery.

Usage:
Upon being alerted of a COVID infected elective surgical patient, our Perioperative Surgical Home (PSH) team used the COVID-COLDS tool to evaluate patient. Based on the score, they were risk stratified. After risk stratification, an OR date was scheduled and formal pre-op evaluation appointment made.

Results:
Using the COVID-COLDS tool, we could safely determine surgical date without increased OR cancellation rate and without increased rate of perioperative respiratory adverse events.

Barriers: Continual changing landscape of CDC and specialty guidance, patient knowledge. Monthly run chart analysis of same day cancellations and respiratory events to ensure safe process.
