Problem: Increase organizational administration of seasonal influenza vaccination to eligible perioperative patients within an independent, fully integrated pediatric health care organization having over 15,000 surgeries per year.
Measurement: We measured weekly rate of eligible patients (>6months old and unvaccinated) receiving influenza vaccine at their Peri-Operative Surgical Home (PSH) clinic appointment. We used run charts to analyze the success of our process.
Analysis: We used the Model for Improvement of quality methodology. Vaccine rate was then analyzed though run charts to measure ongoing PDSA cycle effectiveness. Results were regularly discussed amongst our team and in partnership with organization influenza committee.
Implementation: Process mapping, SFMEA, KDD helped identify obstacles: department culture, hospital policies, vaccine supplies/storage, and new EMR requirements. Multidisciplinary teams and evidence based review achieved interval buy-in. Shared medication and supply use with neighboring department and protocolization of screening, administration, and documentation were implemented. Staff engagement and EMR development apply broadly.
Results/Discussion: We exceeded our initial goal rate of 10% with 10.8% Year1. Learning from Year1 enabled process updates and achieved 15% rate Year2. Run chart analysis provided real time data for PDSA updates, KDD analysis identified additional opportunities. Program will expand to additional campuses based on this success.
