Tiered Safety Huddle: Strengthening Human Connections, Communication and High Reliability

Tiered Safety Huddle.
This huddle system was structured daily to create escalation of issues from the frontline, to the hospital level and then to the system level for resolution and back down to the frontline.

Problem:
The pandemic brought us into a situation of rapid change with communication, collaboration, and urgent action key to successful operations. We needed a daily structure that would allow for timely escalation/resolution of issues and information sharing.

Tool Selection:
Our approach leveraged best practices from other industries and combined them with internal design ideas to create a unique format to the safety huddle practice. These huddles take just 15 minutes and allow all leaders across our organization, including hospitals, physician clinics, and ambulatory departments to develop situational awareness.

Usage:
Staff intentionally pause for huddle daily, to review what has occurred over the last 24 hours and report up through a tiered approach any serious safety events that occurred, any safety issues that cannot be resolved with local resources, and any shared local learnings to cascade across the organization.

Results:
Reports of safety events across hospitals in the system increased by 58% and near-miss reporting increased by 81%.; 3,523 total escalated events have been reported up through the Tiered Huddle System where additional resources were needed to support resolution. Critical to success was the daily structure and intentional follow up.
