Heart Failure Navigator – North Kansas City Hospital
Patricia Sanchez has been the Heart Failure Nurse Navigator at North Kansas City Hospital since its inception in October 2019. She has been instrumental in the development of the role, its processes, and researched best practices to improve patient outcomes. She shows her compassion for heart failure education through her work in a variety of areas of the hospital including projects to update and consolidate heart failure patient education, multidisciplinary readmission reduction team, and collaboration with the Heart Care Clinic. Sanchez began her career at North Kansas City Hospital 22 years ago as a unit secretary and has continued her dedication to the organization through her professional growth serving in a variety of roles including certified nurse’s assistant, medical surgical nursing, home health case management, outpatient clinics, and case management for heart failure and stroke patients. She received her Bachelor’s in Nursing at University of Central Missouri School of Nursing and achieved certification as a Certified Medical-Surgical Registered Nurse.
