Jenny Bernard, DNP, MSN, RN-BC, AGNP-BC

Jenny Bernard is a nurse practitioner and a quality improvement leader who serves as the HMH network’s director for community health quality improvement. She earned her BSN/MSN from Rutgers University and her doctoral degree from Monmouth University. She is an advanced practice nurse with experience in multiple diverse care sites, where she has led multiple quality improvement projects to address equity and increase access to care for underserved populations. During her 20 years, she has been committed to working with low-income, underserved communities. Dr. Bernard is the founder of The First Thirty Program, a transitions-of-care program that provides equitable care to underserved populations across ten acute care hospitals, which decreased the readmission rate by more than 50% with a 98% patient satisfaction rate. She has been the recipient of multiple awards for her commitment to serving underrepresented communities.
