Assessing Progress on the Journey to High Reliability

As VHA works to continuously improve its HRO assessment process, it identified opportunities to optimize the baseline version of the VHA HRO Framework, Maturity Model, and Matrix for usability across the enterprise. This improvement cycle included a literature review and an exploration of best practices. In addition, this work sought to create alignment with other existing frameworks. A gap analysis was done to develop the next iteration of the Framework, which was piloted across various settings throughout VHA prior to broader implementation. This session will describe the development of the VHA Framework and the improvement cycle used to optimize it.

Problem: How do you continually assess progress in high reliability?

Measurement: An HRO Maturity Matrix tool was created to identify level of HRO maturity according to 14 HRO focus areas.

Analysis: Literature review was conducted for development and PDSA cycles were used for testing.

Implementation: Completion and submission of the HRO Maturity Matrix was implemented across the VHA enterprise. As the largest health care system in the US, one of the largest obstacles was the size of the implementation. As a part of the process the team created a toolkit, resource guide and Q&A sessions.

Results/Discussion: As a result of this process, each facility within VHA is now able to reflect on their HRO maturity status and plan HRO activities to support progress in their maturity status around the 14 focus areas covered by the HRO Maturity Matrix.
