Jazmin Cascante, APN, has been a nurse for 21 years at Hackensack Meridian Health – HUMC. Her experience ranges from Clinical Level III RN, Nurse Manager, and as an APN for the last ten years. As a Nurse Practitioner she was the lead Coordinator in the DSRIP/Transitions of Care (TOC) program and is currently the Clinical Lead Manager in the Quality Improvement Program-NJ (QIP-NJ). She earned her BSN from Felician College, and her MSN from Rutgers University, and is currently pursuing her DNP degree. She received her APN and Med-Surg certification from the American Nurses Credentialing Center (ANCC) and is affiliated with the National Association of Hispanic Nurses (NAHN) and the American Association of Nurse Practitioners (AANP). She has led multiple quality improvement projects in Patient Satisfaction (HCAHPS) and Falls Reductions. She has been a poster presenter on Transitions of Care and Transforming Care at the bedside for the Institute for Health Improvement (IHI) National Conference. And has extensive experience working with underserved cardiac, behavioral health, and maternal health patients.
