Leigh Starr, MHA, FACHE, LSSBB, CMP

Director – VHA High-Reliability Enterprise HRO Support Team Department of Veterans Affairs

Leigh Starr serves as director of the VHA High-Reliability Enterprise Support office, leading support for VHA’s High-Reliability Organization (HRO) transformation, as aligned with VHA Quality and Patient Safety. She joined VHA Central Office in 2010 in the VHA Office of Systems Redesign and Improvement, serving as an associate director and the VHA Transformation 21 (T21) Systems Redesign Sub-Initiative Lead, and later served as the office’s deputy director. Starr has held operations and quality leadership positions at the VHA network and medical center levels. Prior to and while at VHA, Starr served as an air force medical service corps officer, retiring in 2020. Among other roles, she served as senior administrator of the 920th Aerospace Medicine Squadron for which she co-led activation. Starr has been an ACHE Fellow for over 15 years and is a Lean Six Sigma Black Belt and certified change practitioner.
